I've been struggling for a while now to figure out how to do this, here's my setup/scenario:
PC running Win 7
2 Denon 3700s as sound cards
Behringer DDM4000 MIDI mixer (I know there are better options but for now... :) )
Issue is if I run the xfader on the mixer in MIDI mode, which is what I want to do to be able to control the audio & video xfader at the same time, when the fader is 100% to deck 1, cue output for deck 2 does not make it to the mixer. This is happening because audio doesn't get sent unless the audio xfader in VDJ is sending audio from that deck. In sound set up, I could set to 3 line mixer and have deck 1 on channels 1&2, deck two on 3&4, and headphones on 5&6, but then under sound card, I only have two cards available (Denon 3700, and 2 Denon 3700) because I'm using the Denon 3700s for the sound card. I would think VDJ would send cue output regardless of where the xfader is.
One note, I haven't installed AISO drivers. With the exception of this, everything is working great. Should I install them, or no?
Any help would be fantastic.

Hi Brian
For vdj to have control over the cue main mix, you MUST use the internal sound setting and absolutely use the Denon asio drivers with thes 37's.
All you really need to do is assign the sound as speakers/phones in box 2, then ASIO in box 3 and assign the channels 1/2 main and 3/4 monitor.
Then on the Behringer find a rotary knob you can use and assign it as the headphone_mix script.

That doesn't seem to work. All kinds of weird stuff going on. You positive this should work like this? I'm getting audio out channels 1/2 no matter where the audio xfader is in VDJ. Very weird...

My results after a little more playing around are:
With Speaker/Phones selected in box 2 and using the ASIO drivers, it appears to work exactly the same as when using an external mixer setting in box two. Very weird. I would think this should work because I did it before with a different set up, but can't figure this out for the life of me. Other than in Box 2, is there someplace I need to indicate channels 3/4 are for headphones?

OK, I figured it out, thank you. My issues started with the wrong ASIO driver for Win 64 bit. Once I changed it, everything just started to fall into place.
